Insurance
We strive to provide quality care while maintaining affordable coverage by accepting insurance and submitting it on your behalf. Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

What is my health insurance coverage and benefits?
What is my deductible and has it been met?
Do I have a co-pay? If so, how much is it?
What is my wellness visit coverage?

Cost, Fees and Payment
Initial evaluations/consultations: $300

Medication management/follow up: $125

Paperwork completion: Please see financial policy

Genetic Testing: There is no charge to collect and submit your DNA sample but costs for the processing of the test itself differ based upon your insurance coverage. A follow up appointment must be made specifically to discuss the results and there is a charge of $25 if you would like a printed copy of your results. 

Cash, check and all major credit cards are accepted for payment, prior to the time of service. Please see our financial policy for charges and fees that may be applicable to your case. To be available to as many patients as possible and to maintain a viable practice, we must ask that you strictly follow our financial policies. 
 

Fees are subject to change dependent upon community standards and fair market value for services rendered.


Cancellation Policy
If you do not attend your scheduled appointment, whether it be in person or virtual, and you have not notified us at least 24 hours in advance, you may be subject to cancellation fees.

Please call the office to schedule an appointment. If it is after business hours or we are currently seeing patients, please leave a message with your best contact information and someone will return your call as soon as possible. If you call after 5:00pm on Friday, non-urgent messages may not be addressed until the following business day.
